About the Role
Hamara Healthy Living Centre (HLC) is seeking a compassionate and motivated Patient Ambassador to support patients within the Burmantofts, Harehills, and Richmond Hill areas. As part of our Personalised Care Team, the Patient Ambassador will deliver Social Prescribing services to patients across 12 GP surgeries within the Burmantofts, Harehills, and Richmond Hill Primary Care Network (BHR PCN).
Social Prescribing empowers individuals to take control of their health and well-being by connecting them to non-clinical support services and community-based resources. This role will involve working directly with patients, assessing their needs, and providing guidance on local services to support their health and social care requirements.
Key Responsibilities
* Patient Support: Conduct holistic assessments of patients' non-medical needs, offering guidance and referrals to local community and voluntary sector organizations.
* Caseload Management: Manage a caseload of patients, ensuring timely follow-ups and efficient use of time and resources.
* Collaboration: Work closely with the wider PCN team, including health professionals and local partners, to ensure integrated care for patients.
* Community Engagement: Build and maintain relationships with local community groups and organizations, developing a directory of resources to support patients.
* Service Delivery: Support patients in setting and achieving personal health goals, with a focus on lifestyle changes and increased community engagement.
* Record Keeping: Maintain accurate and confidential patient records in accordance with practice guidelines, contributing to case studies and reports to demonstrate impact.
* Professional Development: Participate in training and development opportunities, including e-learning and shadowing, to ensure high-quality service delivery.
Person Specification
Essential:
* Experience in health, social care, or voluntary sector roles.
* Knowledge of the wider determinants of health and health inequalities.
* Strong interpersonal and communication skills, with the ability to engage people from diverse backgrounds.
* Ability to assess patient needs and signpost to appropriate services.
* Empathy and non-judgmental approach to supporting vulnerable individuals.
* Strong organizational and time-management skills.
* Ability to work independently as well as part of a team.
Desirable:
* Experience in primary care or as a Social Prescriber.
* Knowledge of SystemOne or EMIS Electronic Health Record systems.
* Ability to speak a community language relevant to the local population.
* Full driving license and access to own car.
* Relevant qualifications (Degree, NVQ level 3, or equivalent experience).
