Job summary Hillton PCN are seeking an experienced advanced nurse practitioner to join our team on a full or part-time basis. The post-holder will be required to act within their professional boundaries, providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for the patients within the general practice. The post-holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC requirements for advanced practice must be met. Whether you have already developed a specific specialty or have an interest in doing so, we will support your ongoing career and training development. Main duties of the job Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad, in-depth, theoretical knowledge-base. Assessing, diagnosing and treating patients conditions, initiating direct referrals, as appropriate. Providing expert advice to patients, carers and colleagues. Developing innovative concepts, models, methods and practices, to help deliver improved primary care to our practice population; Advancing patient health and well-being through the use of health promotion, health education, screening and therapeutic communication skills. As an independent prescriber: to prescribe safe, effective and appropriate medication, as defined by current legislative framework; Leading and undertaking research and audits. Providing leadership, education and training to different healthcare professionals, staff and students.
