Adults Hospital Discharge Social Worker (Locum)
£38 per hour, 6‑month initial contract.
Hampshire County Council are recruiting for a Social Worker in their Hospital Discharge Teams based out of Queen Aexander Hospital, Portsmouth.
Role will involve triangulating, resolving issues in a fast‑paced environment, safeguarding, managing expectations, speaking with patients and their next of kin, and managing a task list.
Responsibilities
* Work with the Advanced Assessment Team (AAT).
* Be part of a multidisciplinary "sub team" supporting early intervention in the hospital to reduce length of stay.
* Complete Care Act assessments and Mental Capacity Act assessments as needed.
* Build professional MDT networks with key areas of the hospital.
* Conduct risk assessments and liaise closely with community teams around "high intensity users" to turn these individuals around.
* Maintain close links with short‑term service and community bed providers to streamline people rapidly to services, avoiding admissions and truncating stays.
* Use HCC systems to provide the AAT with collateral information for rapid decision‑making.
* Represent the AAT on wards, including participation in stranded patient sessions if required.
