What You Will Do:
Carry out assessments and reviews for children in care, ensuring effective and responsive service delivery.
Develop tailored care plans in collaboration with children, families, and carers, addressing the unique needs of each child.
Liaise with multi-agency partners to support and safeguard children’s health, education, and overall development.
Maintain accurate, up-to-date records in line with statutory requirements and best practices.
Actively participate in team meetings, supervision, and developmental workshops.
Provide practical guidance and emotional support to children, carers, and families, signposting them to relevant resources when appropriate.
