Responsibilities:
1. Assess and review individual and carer needs, initiating and coordinating solutions/interventions to meet personalized outcomes.
2. Identify potential reablement opportunities and facilitate access to relevant services.
3. Promote independence and personalization, enabling service users and carers to assess and manage their needs, risks, and uncertainties within their living environment.
4. Identify community and other support resources, maximizing individuals' assets, preventative/universal services, and funding sources, in line with the Adult Social Care Strategy and the concept of 'just enough' support.
5. Review ongoing service provision to ensure all alternatives are considered.
6. Provide information, advice, and support to service users and their carers.
7. Utilize the Mental Capacity Act practice framework as necessary.
8. Support Safeguarding Officers and Managers during enquiries, performing tasks that complement their work.
9. Manage your social care workload independently with appropriate oversight.
10. Undertake assessments using health and social care tools following training, such as contributing to continuing health care assessments.
11. Liaise and negotiate with local providers and support networks to improve outcomes.
12. Contribute to practice and service development, working flexibly as needed.
13. Maintain confidentiality and handle information responsibly.
14. Possess a full driving license and access to transport; disabled employees unable to drive may use taxis.
15. Work flexibly and efficiently to meet objectives and targets.
