Job Summary

Halton Community Therapy Team is advertising for a Band 4 Assistant Practitioner to work within HCTT, and with an interest in Orthopaedics.

HCTT is a team of Physios & OTs working in the Community setting of the Borough of Halton - which involves Assessment & Treatment of adult patients in their own home.


Main duties

The successful applicant would be expected to complete Initial Assessments & devise follow up plans and treatment as a lone worker & car driver. The majority of your daily duties will be the assessment & treatment of Total Hip Replacements discharged from local hospitals; but this may also include simple mobility assessments and to assist more senior therapists with more complex patients.

To support Admin staff with Registration of new referrals onto the computer system.

Job responsibilities

The Assistant Practitioner Physiotherapist will work as part of a multidisciplinary team to deliver high-quality, patient-centred care across community settings. The post holder will manage a delegated caseload and defined client group, working autonomously within agreed protocols and under the supervision of a qualified clinician. They will support the assessment, treatment, and evaluation of patients, contributing to the delivery of therapy programmes that promote functional independence, mobility and wellbeing. The duties will include maintaining accurate, clear, timely documentation of patient contacts, progress notes, equipment records and statistical data, in line with Trust policy.


Person Specification


Essential

* Worked as Senior therapy Assistant with in depth knowledge of therapeutic principles.
* Working without direct Supervision.


Desirable

* Experience of working with MDT
* Understanding of therapy roles.


Essential Qualifications

* Foundation Degree in Health & Social Care/BTEC or equivalent Level 5 qualification in a Health & Social care subject.


Desirable Qualifications

* Care Certificate.


Essential Skills, knowledge and competencies

* An understanding of human anatomy & physiology, and how they impact on the individuals functional independence/mobility.
* Ability to work under pressure, and ability to prioritise.
* Ability to develop effective interpersonal relationships with colleagues across a number of disciplines.


Desirable Skills, knowledge and competencies

* Able to follow instructions and procedures.
* Able to work in team and under own initiative.
