Older Persons Community Mental Health Nurse
We are looking for an experienced and caring nurse to work within a multi‑disciplinary team (MDT) focusing on older persons living in the community who suffer a wide range of mental health problems.
Responsibilities
* Assess client needs and develop individual holistic care plans, working in partnership with other professionals.
* Plan and implement care packages and provide education, guidance and support to student nurses, unqualified staff and clients’ families.
* Act as care coordinator for a designated caseload, ensuring mental health measures, care management protocols and standards are adhered to.
* Provide evidence‑based assessment of clients’ physical, psychological, spiritual and social needs, taking into account ethnicity and other cultural factors.
* Provide education and support to students and staff and deliver regular monitoring and recording of clients’ physical observations and progress.
Person Specification
Qualifications
* Registered Mental Nurse (RMN) with a Diploma Level or equivalent education.
* Desirable: broad range of experience in health and social care settings.
Skills
* Essential: Ability to prioritise tasks, work proactively and manage a caseload without direct supervision.
* Desirable: IT skills, phlebotomy skills and monitoring of clients’ physical observations.
Personal Qualities
* Essential: Clear and accurate communication, ability to adapt to new and changing circumstances, self‑motivated.
* Desirable: Commitment to continuous improvement of client care.
Knowledge
* Essential: Good understanding of mental health problems in older persons, awareness of current developments in the clinical setting and completed training in mentoring and assessing student nurses.
* Desirable: Awareness of current developments within mental health services for older people and understanding of clinical governance.
