Nurse Practitioner (Band 6/7)
Contract: Full or Part-time
Location: Across Southampton – PCN clinics, community settings & home visits
Salary: £25–35 per hour (depending on experience) + NHS pension + indemnity cover

The role
We are looking for experienced, motivated Nurse Practitioners to join our dynamic, multidisciplinary team. You will work across practices and in the community, carrying out clinics, home visits, and assessments that make a tangible difference to patients' lives.
What you'll do
* Run practice and community clinics, including CGAs, LTC, SMI & LD reviews
* Develop and review personalised care plans
* Support opportunistic health promotion and proactive population health interventions
* Use TPP SystmOne to deliver joined-up care
What we're looking for
* Independent Nurse Prescriber / Nurse Practitioner (history-taking, examination, LTC reviews, CGA)
* Primary care experience desirable
* Full UK driving licence essential
