Paramedic practitioners work independently within theirscope of practice in the community, using their enhanced clinical assessmentand treatment skills, to provide first point of contact for patients presentingwith undifferentiated, undiagnosed problems to assess, and manage patientspresenting with acute presentations which include minor illness, or injury,abdominal pains, chest pains and headaches. They are health professionals whopractice at an advanced level having the capability to make sound judgements inthe absence of full information and to manage varying degrees of risk whenthere are complex, competing or ambiguous information or uncertainty.

Assessand triage patients, including same day triage, and as appropriate providedefinitive treatment [including prescribing medications following policy,patient group directives, NICE [national] and local guidelines and local carepathways] or make necessary referrals to other members of the primary careteam.
Advisepatients on general healthcare and promote self-management where appropriate,including signposting patients to the PCNs social prescribing service, andwhere appropriate, other community or voluntary services.

Responsibilities underpinning the role

Performinvestigatory procedures as required, and undertake the collection ofpathological specimens including intravenous blood samples, swabs, and othersamples within their scope of practice, and within line of local and nationalguidance.
Supportthe delivery of anticipatory care plans and lead certain services (e.g.monitoring blood pressure and diabetes risk of elderly patients).
Providean alternative model to urgent and same day GP home visit for the network.
Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis, and patients withcomplex needs
Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly
Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare
Diagnoseand manage both acute and chronic conditions, integrating both drug- andnondrug-based treatment methods into a management plan

Maintainaccurate and contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associated withpatient health care and registration with the practice.
Communicateeffectively with patients, and where appropriate family members and theircarers, where applicable, complex and sensitive information regarding theirphysical health needs, results, findings, and Job treatment choices
Planand organise own workload, including audit and project work
Torecord personally generated information and maintain a database of informationrelating to the work done in the practices. Personally generated informationincludes information and records relating to audit and clinical work undertakenby the post holder, reference notes relating to clinical/technical information,etc. The database includes maintaining up-to-date, detailed records of all workdone in the practices for which the post holder is accountable (done by thepost holder or others)
Tomaintain registration as a Paramedic and comply with appropriate professionalcodes
Asappropriate to the post, to maintain and develop professional competence andexpertise, keep up to date with medical/therapeutic evidence and opinion, andlocal and national service, legislation and policy developments, agreeobjectives and a personal development plan and participate in the appraisalprocess
Toattend local, regional and national meetings of relevance by agreement
Toundertake any other duties commensurate with the post holders grade as agreedwith the post holders line manager

Codes of Conduct
Paramedics must comply with theCode of Professional Conduct set by the Health and Care Professions Council[HCPC] and to Rochdale Health Alliance Code of Conduct.
Person Specification
Experience
* Have completed a 2 years Consolidation of Learning period as a newly qualified paramedic
* Have completed a further 3 years experience as a Band 6 [or equivalent] paramedic
* Evidence of enhanced clinical assessment
* An appreciation of the nature of GPs and general practices
* Excellent interpersonal, influencing and negotiating skills
* Excellent written and verbal communication skills
* Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
* Able to obtain and analyse complex technical information
* Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or
* GPs when appropriate
* Able to work under pressure and to meet deadlines
* Produce timely and informative reports
* Gain acceptance for recommendations and influence/motivate / persuade the audience to comply with the recommendations/ agreed course of action where
Qualifications
* Degree in Paramedicine or equivalent experience
* Working towards developing Level 7 capability in paramedic areas of practice and has completed and signed off formally within the clinical pillar competencies of Advanced Clinical Practice Framework
Professional Registration
* Mandatory registration with the Health and Care Professions Council
