An exciting opportunity has arisen within the Integrated Care Delivery Service, and we are recruiting an Admission & Discharge Coordinator to work alongside our team at Dolphin Manor Care Home in Rothwell

About you
We are looking to recruit a well motivated and dynamic individual to work as part of the team at Dolphin Manor Specialist Short-Term Dementia Service. You will take responsibility for the coordination of all admissions and discharges, ensuring prompt action is taken to address any factors which could cause a delay in a person’s discharge home.
You will have excellent communication skills and be able to successfully represent our priority to reduce unnecessary length of stay and prevent delayed discharges.
The successful candidate will demonstrate skills, knowledge and experience in the following key areas of responsibility:
1. Support the safe admission processes, including responsibility for medicine reconciliation and transcription.
2. Highlight as early as possible people who are ready for discharge and work with the MDT to coordinate and facilitate their onward move.
3. Be an advocate of strengths-based social care and for promotion of approaches to maximise independent living and championing the city-wide Home First ambition.
4. Coordinate referrals to system partners and external organisations such as Homecare Providers, Care Homes and Neighbourhood NHS teams and Active Recovery.
5. To update the Multi-Disciplinary Team on the progress of patient discharges and any delegated actions.
About the role
Dolphin Manor is delivered in partnership with NHS organisations in Leeds, providing specialist short-term 24-hour care to people with Dementia. The service focusses on recovery of wellbeing for people experiencing psychological distress and behavioural needs.
