Social Worker - Adults Locality Team
This role offers an exciting opportunity to contribute to a team dedicated to supporting adults within a locality team.

* Conduct comprehensive assessments of adults needs and develop person-centered care plans.
* Provide direct social work services, including counselling, case management, and advocacy.
* Collaborate with other professionals, such as health care providers and family members, to coordinate care.
* Maintain accurate and up-to-date case records and documentation.
* Participate in team meetings and contribute to the development of policies and procedures.
* Engage in community outreach and education initiatives.
* Advocate for the rights and well-being of adults in need.
* Monitor and evaluate the effectiveness of interventions and make necessary adjustments to care plans.
Requirements
We're looking for you, if:
* You hold a relevant social work qualification (e.g., MSW, LCSW).
* You possess a minimum of 2 years of experience working with adults in a social work capacity.
* You have a strong understanding of social work theories and practices.
* You are proficient in conducting needs assessments and developing care plans.
* You possess excellent communication, interpersonal, and problem-solving skills.
* You are a highly organized and detail-oriented individual.
* You are able to work independently and as part of a team.
* You possess a valid driver's license and access to a reliable vehicle (if applicable).
* Experience working within a locality team is advantageous.
