The nursingteam lead will:
Beresponsible for the clinical management of the Intermediate Care Nursing Team,ensuring effective and efficient delivery of a high quality timely service tomeet commissioner requirements.
To undertakehighly complex clinical assessments and ensure the nursing team provides carein line with Trust and professional body standards.
Plan capacityand demand to deliver a safe service and contibute to admission avoidance and atimely discharge home.
Motivate andsupport the nursing team, inclusiveof supervision and staff development toensure a high standard of care and support to patients.
Carry out theduties ensuring the service promotes equality and diversity, offering person centred care with dignity andrespect.
To beresponsible for the implementation of partner agency and Trust policies andprocedures.

Job responsibilities
Leadership & Management
Towork collaboratively with Intermediate Care Service and Stakeholders toensure patients needs are met
Toprovide leadership and support by promoting professional development andencouraging clinical supervision
Ensure that capacity isproactively managed to meet the demands of the services
To promote good team workingand good working relations amongst staff.
Toensure timely accurate information is provided on annual leave, expenses andstudy leave
Tomonitor and report to line manager all HR, safeguarding and health and safetyissues discussing and taking action as appropriate
Toactively manage sickness and absence of staff accessing support from theirline manager as applicable
Toensure effective use of physical and financial resources including theordering of supplies and equipment
Tobe responsible for the equitable distribution of rotas and prioritisation ofwork load
To monitor, report, audit andevaluate that staff maintain appropriate and accurate patient records
Tooversee the collection of all monitoring data and contribute to performancemanagement
Tolead on the clinical aspect of complaints, taking action where appropriate toensure learning as a result of patient/service user feedback
Investigateincidents relating to nursing practice, ensuring that learning takes placeacross the service,
To assist their line manager to recruit, motivate, develop supportand induct staff with policy andservice priorities
Responsiblefor own time management and that of the nursing team
Flexibility to participate inrota cover to meet the service needs
To manage time to ensure aspilt of 80% leadership/management and20% clinical duties to ensure maintenance of clinical skills
ClinicalResponsibilities
To complete comprehensive assessment of highly complexpatient nursing needs. Plan, assess, implement and evaluate care deliveryaccording to changing health needs using prescribing skills
Toensure the nursing teams practice is evidence based through undertaking auditand critically appraising literature to achieve a high standard of care
Toactively contribute to multidisciplinary meetings
Toestablish and maintain effective communication with individual groups andcommunities in complex and potentially challenging situations
Toinvolve patients and carers in the planning and delivery of care
Tocompetently undertake as required arange of clinical skills related to the post
Tomaintain accurate records in accordance with NMC and Care Quality Commissionstandards.
Toidentify skills and knowledge to address self and individuals training needsby undertaking PDR
Todemonstrate the ability to work without direct supervision and to solveproblems in a collaborative and inclusive manner

Lead audit activity as set outin the service quality assurance framework to ensure high quality care ismaintained and where required, learning is embedded.
To offer advice and support toothers as appropriate, acting as a positive role model, for all staff.
Where required and in line withTrust policies, address shortfalls in performance, escalating concerns to theservice manager
To lead and participate in protected learning time and attend staffmeetings as requested.
Tomaintain competency to practice through Continuing Professional Development(CPD).
To contribute to own CPD in apositive manner, evaluating ones own practice and
participating in the appraisalprocess. Attend relevant training courses and
mandatory training.
To cascade policy documentswithin the team ensuring their implementation and understanding.
To positively represent andpromote the Intermediate Care Service.
To lead and engage as requiredin specific projects, meetings and work arising from the responsibilities ofthe team manager in the wider delivery of service provision.
Lead the teams professionaldevelopment and participate in individual Performance Development System
