Overview
We are seeking a Triage Practitioner to act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols. We are Kent and Medway Mental Health NHS Trust, an organization rated ‘Good’ by the Care Quality Commission, supporting children, young people and adults with a wide range of mental health needs.
Responsibilities
* Complete brief 20‑30 minute tele‑triage calls to establish needs and risk and to make a clinical decision on whether a referral is accepted into Children & Young People’s Mental Health Services or sign‑posted to primary care.
* Screen referrals coming into the service, acting as the primary gate‑keeping measure to separate emotional‑well‑being referrals from those requiring further triage.
* Complete IT tasks, including recording in the Electronic Patient Records System and proficient use of Microsoft Outlook, Excel and Word.
* Act as an autonomous, registered practitioner accountable for unsupervised actions, guided by the professional code of conduct and Trust guidelines and protocols.
* Be responsible and accountable for service delivery to clients via NICE‑compliant pathways and in accordance with the crisis care concordat.
* Initiate referrals to other health‑professional specialist services and agencies.
Qualifications
* Graduate‑qualified health care professional (e.g. nurse, social worker, occupational therapist) or substantial Children and Adolescent Mental Health Services (CAMHS) clinical experience.
* Fully registered with the appropriate professional or regulatory body (e.g. HCPC, NMC).
* Extensive clinical experience working with children and young people with emotional wellbeing and mental health difficulties.
