The Role As a Care Manager, you’ll play a central role in overseeing the safe, effective and person‑centred delivery of care across our services, ensuring people receive the right support, at the right time, in their own homes. Your responsibilities will include: · Managing a caseload of clients, ensuring care plans, risk assessments and reviews are accurate, up to date and outcomes‑focused · Providing leadership, guidance and support to care staff to maintain high standards of care · Carrying out assessments, reviews and quality monitoring visits · Acting as a key point of contact for clients, families, care teams and external professionals · Ensuring care delivery remains compliant with CQC standards, safeguarding requirements and company policies · Identifying concerns early and taking appropriate action to maintain safety and quality · Supporting service development, audits and continuous improvement activities As a Care Manager, your professional judgement and leadership will make a real difference. About You You’ll be a confident, organised and compassionate professional with a strong background in care or healthcare. This role is ideal for someone with previous clinical or healthcare experience — for example a Registered Nurse who is no longer practising, or a Physiotherapist or Occupational Therapist would be ideal.
