The successful post holder will join a team of Occupational Therapists, Apprentices, and Assistants covering Medical, Surgical, and Respiratory wards. They will support patients, families, and carers in planning for home after hospital. We are a supportive, welcoming, and passionate team that works together for the benefit of each other, our patients, and the wider team. We value making new team members feel capable and confident, and therefore offer a good induction, in-service training, and opportunities for further learning. Regular support is provided through supervision, daily caseload meetings, and monthly team meetings.
Support the clinical delivery of Occupational Therapy on the Acute Wards by:
1. Prioritising patients who require OT to support weekend discharges.
2. Providing interventions including assessing and providing equipment to enable and support discharge.
3. Carrying out Risk Assessments for care agencies for new equipment.
4. Signposting and referring patients to services to support follow-up after discharge.
5. Liaising with the MDT to resolve issues blocking weekend discharges, such as working with transport and discharge support teams.
6. Providing guidance and training to patients, families, and carers to promote independence at home following acute illness.
