We have an exciting opportunity for a Social Worker (30 hours per week) to join the Transfer of Care Hub Social Worker team. The Transfer of Care Hub is an integrated health and social care team co-located at Blackpool Teaching Hospitals to support people to be discharged in a safe and timely manner.
The role requires experience and interest in crisis intervention and safeguarding, with the ability to make robust evidence-based and legally literate decisions to support people and their families navigate hospital discharge and access onward care and support services in the community. A strengths-led approach to care and support planning, ensuring the person bvoice is heard, is essential. You will be passionate about supporting Blackpool residents.
Responsibilities
* Apply a strengths-led approach to care and support planning, ensuring the person’s voice is heard.
* Support people and their families to navigate hospital discharge and access onward care and community services.
* Make robust, evidence-based and legally literate decisions in collaboration with the care team.
* Work as part of the Transfer of Care Hub to enable safe and timely discharges.
* Collaborate with colleagues and managers, with access to regular supervision and development opportunities.
* Operate on a 7-day rota with advance notice, paid at an additional increment.

* Commitment to ongoing professional development with supervision and training opportunities available.
