This role is ideal for a registered nurse with a passion forproactive, personalised care, collaborative working, and developing the futureworkforce.
Working across our networks practices and communities,you will provide clinical leadership and mentorship, contribute to populationhealth initiatives, and support our Care Home & Frailty team. You will be akey part of our neighbourhood-based, multidisciplinary approach to healthcaretransformation.

SWAN Primary Care Network constitutes 7 GP practices, located in South Wigan Ashton North area, with a collective patient population of around 35,500. The member practices are Marus Bridge Practice, Medicentre, Bryn Cross Surgery, South Wigan Medical Practice, Winstanley Medical Centre, The Grange Practice and Kumar Family Practice.
