You have a strong commitment to mental health advocacy and the well-being of those in your care. Having your own transport is essential, as you will need to travel around and outside the borough as required. What the Job Involves As a key member of the LB Sutton Mental Health team, you will be primarily responsible for managing hospital discharges from mental health facilities. Your role involves conducting statutory Care Act tasks, including assessments, reviews, and care planning. You will work closely with multidisciplinary professionals, participate in ward rounds, and engage with housing and brokerage services. Additionally, you will handle Court of Protection applications, attend Mental Health Tribunals, and carry out safeguarding enquiries and duty work. This is not a Care Coordination role, but rather a specialized position within the local authority's mental health social work team.
