Care Plus Group provides person centred support to vulnerable adults requiring specialist therapy input within the area. We have a number of therapy rotations, each area allowing for specialist development and unique opportunities to contribute to our therapy team. Please see additional information for more details.

* Intermediate Care @ Home
* Community urgent care team
* Moving and handling
* Community In-patient Unit
Within each area there is highly skilled practitioners that place the individual at the forefront of their care and promote faster recovery from illness, prevent unnecessary acute hospital admissions and premature admissions to long‑term care, support timely discharge from hospital and maximise independent living wherever possible. This is achievable through creative and unique interventions within each rotational area. Multi‑professional staff will collaborate with individuals to ensure they achieve their full potential in terms of their level of independence, health and wellbeing.

Main duties of the job
Rotational Occupational Therapist – Permanent (37.5 hours per week) – Salary Range £30,805 - £35,715 per annum. Vacancy based at: Various Locations – Rotational Post, North East Lincolnshire.
We are looking for an enthusiastic and dynamic Occupational Therapist to join our well‑established therapy teams in North East Lincolnshire. This is a great opportunity for newly qualified Occupational Therapist with appointment dependent on successful completion of course and HCPC Registration.
You will be expected to provide a high standard of practice, undertaking all aspects of clinical duties as an autonomous practitioner.
You will liaise with the wider MDT and participate in the provision at each rotation in a variety of locations with the community, including home visits and group settings, providing an effective rehabilitation and discharge preparation service.
You will manage a defined caseload of patients using evidence‑based / patient‑centred principles to assess, plan, implement and evaluate interventions, including discharge planning.

Qualifications
* Degree/Diploma/MSc in Occupational Therapy
* HCPC Registered
* Member of RCOT
Experience
* Application of the OT process
* Use of functional assessment and activity analysis during clinical placement
