This is an ideal position for a nurse with a minimum of five years post-registration experience.
Main duties of the job
The post holder will be responsible for delivering nursing services, working as part of the practice multidisciplinary team, and providing care within their scope of practice to the entitled patient population. The practice nurse will manage clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, and support the management team in reviewing clinical policies and procedures.

Job Title: Practice Nurse/Specialist Nurse/Community Matron
Brief Description:
Responsible for ensuring the delivery of safe and effective nursing care, including treatment, preventative care, screening, assessments, home visits, and patient education to the entire practice population. The post holder is accountable for nursing service delivery and will work closely with the Lead Nurse and GPs to deliver practice priorities.
Main Duties and Responsibilities:
* Meet the nursing needs and provide nursing care to patients, including general and specific health assessments and screenings, independently and in consultation with GPs.
* Demonstrate the rationale and evidence behind methods chosen to meet healthcare needs.
* Participate in practice policy development and provide nursing input into patient care decisions. Keep clinical management protocols up to date, conduct audit cycles, and liaise with the Lead Nurse and Practice Manager regarding deadlines.
* Contribute to administrative and professional responsibilities within the practice team.
* Review and implement infection control measures and update clinicians at meetings.
* Participate in education, training, teaching, and support of students and staff of all disciplines within the practice.
* Maintain professional development through courses and study days.
* Maintain accurate patient records and statistics, developing IT skills to support clinicians.
* Control and maintain medical supplies and equipment, including sterilization and inventory management.
* Advise patients on general health care and minor ailments, referring to GPs as necessary.
* Provide women's health services, including cervical screening, breast screening advice, Chlamydia testing, contraceptive checks, and signposting for contraception consultations.
* Assess, diagnose, and prescribe within your capability and nursing grade.
* Care for and educate patients with chronic diseases, especially diabetes and hypertension, following NICE guidelines and QOF. Includes home visits for housebound patients.
* Manage wounds, removals, injections, venepuncture, microbiology result interpretation, and follow-up treatments.
* Perform compression bandaging, hosiery assessments, Doppler, and Monofilament assessments. Develop wound care plans and refer to TVN as needed.
* Take cervical smears, maintain records, issue results, and follow up referrals. Contribute to cytology recall systems.
* Lead on childhood immunizations, ensuring accurate record-keeping and efforts to improve uptake.
* Handle telephone inquiries, EMIS tasks, and advise on nursing queries, including appointment booking and specimen preparation.
* Prioritize health problems and intervene appropriately in urgent or emergency situations, including initiating emergency care.
* Complete EMIS alerts during consultations.
* Chaperone patients when necessary.
* Undertake basic observations, smoking cessation advice, alcohol intake counseling, BMI, blood glucose monitoring, ECGs, and frailty scoring.
* Perform home visits and assessments.
* Conduct ear assessments, hearing checks, and ear irrigation if indicated.
* Participate in NHS Health Checks, health promotion, disease prevention, and review blood results.
* Complete EMIS templates with individualized information, care plans, and follow-up details, sharing with patients for continuity.
* Perform any other duties appropriate to the role.

Qualifications
* Diploma or Degree in Nursing
* Registered with the Nursing and Midwifery Council (NMC)
* Qualification in Chronic Disease Management - Respiratory
