Job Summary
We're looking for a dedicated and compassionate Support Worker to join our team, supporting two males in their 30s who live together in a shared home. Both individuals have complex mental health needs and benefit from structured, consistent support tailored to their unique circumstances.
Responsibilities
You will provide consistent, person-centred support to two individuals with complex mental health needs, prompting with daily living tasks such as personal care, meal preparation, medication, and household duties, while promoting independence, choice, and dignity. You'll build positive relationships, encourage engagement in routines, monitor changes in health and behaviour, and respond appropriately, including supporting individuals with English as a second language using clear, simple communication.
You will maintain accurate records, work with professionals and families for coordinated care, support physical health needs including diabetes management, and always follow safeguarding, health and safety, and confidentiality policies.
One of the gentlemen has a diagnosis of Paranoid Schizophrenia with autistic traits (undiagnosed) and may engage in substance or alcohol use, which can occasionally lead to verbal aggression. The other has diagnoses of Schizophrenia, ADHD, and Type 2 Diabetes. He has a history of extended hospital admissions and requires ongoing support with communication, self‑care, and managing both his mental and physical health. His first language is Polish, so clear, straightforward communication is essential, particularly during periods of low mood. Cannabis use may also impact his mental health.
