Offer description

The Inreach and Home Treatment Team, based in Chesterfield, is seeking a dedicated and enthusiastic Support Worker to join our welcoming and supportive team.

This role offers a rewarding opportunity to work with older adults experiencing a deterioration in functional mental health. The service operates 9am to 5pm, seven days a week, including bank holidays, so flexibility is essential.

If you are passionate about making a difference in older people's mental health and thrive in a dynamic, community-based setting, we would love to hear from you.

* Support service users in their homes or other agreed settings.
* Assist senior staff in implementing and reviewing care plans by feeding back your findings/thoughts.
* Encourage participation in therapeutic activities.
* Support practical tasks outlined in care plans.
* Report concerns to appropriate professionals.
* Maintain accurate records.
* Work flexible hours, including weekends.
* Travel across a defined geographical area as needed which may include cross working if required.
