The Inreach and Home Treatment Team, based in Chesterfield, is seeking a dedicated and enthusiastic Support Worker to join our welcoming and supportive team.
This role offers a rewarding opportunity to work with older adults experiencing a deterioration in functional mental health. As part of a multidisciplinary team, you will contribute to delivering intensive, home-based treatment as an alternative to inpatient care. Under supervision, you will provide competent and compassionate care tailored to individual needs.
The successful candidate will be required to travel across a wide geographical area, covering Chesterfield and North East Derbyshire, and therefore must hold a valid driving licence and have access to a vehicle. The service operates 9am to 5pm, seven days a week, including bank holidays, so flexibility is essential.

* Support service users in their homes or other agreed settings.
* Assist senior staff in implementing and reviewing care plans by feeding back your findings/thoughts.
* Encourage participation in therapeutic activities.
* Support practical tasks outlined in care plans.
* Report concerns to appropriate professionals.
* Maintain accurate records.
* Work flexible hours, including weekends.
* Travel across a defined geographical area as needed which may include cross working if required.

NVQ Level 3 in a care-related field or equivalent.
Basic computer literacy.
Ability to travel and work flexibly.
