Reference 115985 Lead Worker Telfor & Wrekin £24706.50 pwr annum 37.5 hours per week full UK driving licence access to a vehicle essential About the role As a Lead Worker, you will provide early, targeted, and flexible support to individuals and families across Telford and Wrekin. You will work with young people aged 16-54 and older individuals aged 55, supporting them to maintain their tenancies, achieve financial independence, and improve their health and well-being. This role focuses on homelessness prevention, tenancy sustainment, and promoting independent living. What you'll be doing Conduct comprehensive assessments of customers' housing, financial, health, and social support needs. Develop personalised, outcome-focused support plans tailored to individual strengths and goals. Collaborate with multi-agency partners to deliver coordinated and holistic support services. Provide practical assistance with budgeting, debt management, and accessing welfare benefits. Support customers in connecting with community services, reducing isolation, and fostering social inclusion. Uphold safeguarding standards, ensuring the health and safety of vulnerable individuals. Maintain accurate records, ensuring compliance with Key Performance Indicators (KPIs). Who we're looking for?
