We are looking for an experienced registered adult nurse who can thrive in our community nursing workforce. You will become an expert clinician in wound care, palliative care, continence care and the care of patients with long term conditions.
Key responsibilities
* Provide skilled nursing care and support the team to deliver high quality integrated care.
* Demonstrate excellent communication and interpersonal skills, be adaptable, reliable, resourceful and approachable.
* Provide integrated care following a holistic assessment enabling patients to be cared for in their own homes and clinic settings.
* Facilitate prevention of unnecessary hospital admission/re-admission by reducing the effects of ill health and disability and maximizing the individual's potential for independence.
* Assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patient's diverse needs, their carers and other professionals.
* Deliver evidence‑based interventions.
* Safely monitor, manage and administer medication.
* Work autonomously, building leadership skills and advanced clinical assessment skills.
* Build effective relationships with patients, their carers, family and colleagues across the MDT.
* Develop exceptional planning and organisational skills.
* Analyse care and treatment provided and develop sound clinical decision making.
* Receive training to deliver high quality care and develop knowledge in all areas including palliative care, leg ulcer management, acute and chronic wound care, bladder and bowel dysfunction and supportive care to carers and family members.
* Delegate and support junior staff, building leadership skills.
Requirements
* Registered adult nurse (Band 5).
* A full UK driving licence and vehicle for business use is required.
* Experience in wound care, palliative care, continence care or long term condition care is desirable.
* Excellent communication, interpersonal and organisational skills.
* Ability to work flexibly, including availability for days off to explore the region.

Additional information
* Applicants are required to pay for their DBS check. The cost (£26.40 standard or £54.40 enhanced) will be deducted from salary over the first 2 months of employment.
* Enrolment for the DBS Update Service incurs an annual fee of £16.
* Employment is 12 months fixed term; the service operates from 8am to 8pm, seven days a week.
