Duties
* Assist patients with personal care tasks, including bathing, dressing, and grooming.
* Support individuals with mobility and physical activities as needed.
* Implement and follow care plans tailored to each patient's needs.
* Monitor and record patients' health status, reporting any changes to the healthcare team.
* Provide companionship and emotional support to enhance the quality of life for patients.
* Maintain a clean and safe environment for patients by adhering to health and safety protocols.
* Utilise IT systems for documentation and communication purposes effectively.
* Drive to various locations as required to provide home care services.
Experience
* Previous experience in a care home or home care setting is preferred but not essential.
* Familiarity with care plans and the ability to follow them accurately is advantageous.
* Proficiency in English is required for effective communication with patients and team members.
* Strong IT skills are beneficial for maintaining records and using healthcare software.
