Role Summary: We are seeking a Level 4 or 5 qualified Assessor, ideally with additional Trusted Assessor qualifications, to join our dynamic health and social care team. This pivotal role involves conducting comprehensive care and support assessments for our service users, ensuring that all care is person-centred, outcomes-focused, and compliant with current legislation and best practice standards. The successful candidate will have proven experience in the care sector at a supervisory level, an excellent understanding of the Care Act 2014, and be familiar with CQC regulations, safeguarding frameworks, and the principles of inclusion and empowerment. A Train the Trainer qualification is also desirable, as you will support workforce development through mentoring and skills-sharing. Key Responsibilities: Carry out holistic care and support assessments, reassessments, and reviews for new and existing service users in line with statutory requirements and service policies Protect the reputational profile of the company when representing us in face to face contact Apply the Trusted Assessor model to streamline minor adaptations and care pathway processes Ensure all assessments are person-centred, strengths-based, and outcomes-focused, with a clear view of individual goals, risks, and aspirations Develop care and support plans that reflect individual needs, preferences, cultural identity, and promote independence and wellbeing Work in partnership with service users, families, advocates, and multidisciplinary professionals including social workers, occupational therapists, and community health teams Monitor and audit assessment outcomes and documentation to ensure CQC compliance, accuracy, and data protection standards. Lead and contribute to multidisciplinary reviews, safeguarding processes, and quality assurance activities Provide supervision, guidance, and support to junior assessors, care coordinators and care/support workers Facilitate staff training, onboarding, and development activities in line with your Train the Trainer capabilities and sector standards Contribute to continuous service improvement through audit, service user feedback, and reflective practice Maintain accurate, timely case records using the organisations Electronic Care Management Systems Access Care Planner Qualifications & Experience: Essential: A proven track record in the sector Car owner and driver Level 4 or 5 qualification in Health and Social Care, or equivalent Accredited Trusted Assessor qualification (or willingness to obtain) Significant experience in conducting needs assessments and developing care plans Strong knowledge of person-centred and outcomes-focused care models Awareness of legislation and compliance standards : Care Act 2014, Mental Capacity Act 2005, CQC regulations Proven team leadership or supervisory experience Excellent written, verbal, and IT communication skills Strong values aligned with dignity, respect, inclusion, and safeguarding Desirable: Recognised Train the Trainer qualification (or willingness to undertake) Experience delivering formal / informal staff training and mentorship What We Offer Full-time working hours (40 hours per week) A supportive and inclusive working environment Ongoing professional development opportunities with dedicated career pathways Opportunity to make a tangible impact on care quality and safety A comprehensive remuneration package that reflects your experience & expertise salary range GBP32,000 to 36,000 per annum A natural ability to multi-task and problem solve in an open and transparent, honest manner is essential, but at the same time, it requires you to have an ability to build and maintain quality, respectful relationships based on integrity and trust, so a fair and equitable manner is required in all instances.
