One of our leading NHS clients is looking for experienced mental health nurses to cover shifts in community hospitals in York and Humber. The right candidate will be enthusiastic and highly organized, capable of providing high-quality nursing care and assessments.
Description:
* The postholder will work closely with a team of doctors, conducting initial patient assessments, ensuring prescribed medication reaches patients, and monitoring their responses.
* Monitor side effects and perform physical checks such as blood pressure screening.
* Assist in assessing and determining patients' mental health needs and potential risks to inform care plans, developing collaborative responses.
* Support follow-up to ensure medication stability and collaborate with the MDT during discharges.
* Allocate weekly patient caseloads and conduct initial assessments.
* Manage patient caseloads, including access to anti-dementia medication and response monitoring.
* Communicate complex bio-psychosocial information empathetically to users and carers.
* Liaise with professionals and agencies to ensure continuity of care through clear and timely communication.
* Gather information as needed and record it accurately in patient records.
* Attend MDT meetings as required for the Memory Assessment Service (MAS).
Skills & Experience:
* Registered Adult Nurse, Mental Health Nurse, or Learning Disabilities Nurse
* Experience managing a ward or clinical unit
* Experience supporting clinical research projects
* Experience in practice development within clinical areas
* Ability to provide responsive, quality care independently
* Resilient and capable of handling difficult interpersonal situations
* Self-aware, confident, motivated, and able to motivate others
* Effective at building professional relationships with colleagues
* Advocate for individuals with protected characteristics; promote equality and diversity
