Job summary
You will be expected to act as autonomous practitioner working independently of and in conjunction with other health professionals to assess, diagnose and treat the conditions of patients attending within primary care, initiating direct referrals as appropriate. You will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice.
Main duties of the job
The post holder will need to prioritise andtriage the needs of patients accordingly making any necessary referrals forinvestigations in the appropriate manner. Patients with problems requiringreferral to secondary care should be referred following practice and hospitalprotocols. If necessary you will provide home visits to the acute unwell patients and housebound CDM patients. You will workcollaboratively with the multi-disciplinary general practice team to meet theneeds of patients, supporting the delivery of policy and procedures. You willneed to maintain a safe and effective level of practice and to bring anydeficits in training to the attention of the practice manager so these can beaddressed

The post holder will need to prioritise and triage the needs ofpatients accordingly making any necessary referrals for investigations in theappropriate manner. Patients with problems requiring referral to secondary careshould be referred following practice and hospital protocols. You will providehome visits to the acute unwell patients and housebound CDM patients on a daily basis. You will workcollaboratively with the multi-disciplinary general practice team to meet theneeds of patients, supporting the delivery of policy and procedures. You willneed to maintain a safe and effective level of practice and to bring anydeficits in training to the attention of the practice manager so these can beaddressed. You will lead the nursing team and mentor and support other nurses in developing and maintaining skills.
