Who You Are You are an experienced agency social worker ready for a six-month placement within the Rhondda Cynon Taf West Learning Disability Team. You have a focus on supporting adults with learning disabilities, fostering an environment of safeguarding and wellbeing, and eager to join a well-established and supportive team. What the job involves Your primary responsibilities will include conducting reassessments and statutory reviews. You will deliver high-quality social work practices such as assessment, care planning, and review processes, adhering to the Social Services and Well-being (Wales) Act 2014. This role involves community-based work with a hybrid model allowing office days. Key tasks include: Safeguarding and promoting wellbeing through assessments and care planning Balanced risk assessment and management promoting independence and choice Identification and enhancement of individual strengths for positive outcomes Collaboration with other professionals and agencies in a multidisciplinary approach Completing and maintaining electronic records in line with departmental standards Skills Proficient in high-quality social work practice Knowledge of Social Services and Well-being (Wales) Act 2014 Ability to assess and manage risks effectively Strong interdisciplinary collaboration skills Proficiency in maintaining detailed records Flexible with travel and hybrid work model
