Due to an internal promotion within the service ESTEP East, a band 5 vacancy based in Rochford is available.


Service Overview

ESTEP is the Early Intervention in Psychosis (EIP) Service that covers South East Essex. We are looking for an exceptional mental health nurse to work as an EIP Care Co‑ordinator within the team. The role is community/office based.


Role Summary

Working with people who have had a psychotic illness, practitioners provide a 3‑year period of engagement to establish relationships, agree care plans and support recovery. The service follows NICE guidelines for care planning and risk management.


Responsibilities

* Manage own caseload in line with C.P.A. procedures and risk management protocols.
* Use evidence‑based and client‑centred principles to assess needs and plan, implement and evaluate health and social care interventions.
* Create and continually update person‑centred care plans and build therapeutic relationships with clients.
* Assess risk and design personalised risk management plans.
* Provide leadership for junior staff through supervision and appraisal.
* Participate in the planning, development and evaluation of MDT services, holding personal responsibility for defined projects and areas of responsibility.
* Mentor and supervise students on practice placements.
* Carry out MDT activities such as coordinating meetings and undertaking duty responsibilities.
* Identify any possible safeguarding concerns and be responsible for the completion of safeguarding investigations.
* Ensure that the requirements of the Mental Health Act 1983 are observed, adhered to and implemented.
* Monitor physical health and promote physical wellbeing.
* Support the promotion of carer and service user involvement within the service and ensure carers’ needs are identified and responded to.
* Provide profession‑specific interventions for the wider team as required and in line with NICE guidelines.
* Participate in team training and mandatory training and ongoing professional development.


Qualifications & Skills

The ideal candidate has existing community experience and a passion for working with this client group. Key skills required include:

* Adapt working approach to establish relationships.
* Balance clinical activity with good standards of record keeping on the electronic record.
* Provide psycho‑education around psychosis.
* Use assessment tools.
* Engage with friends, families and carers.
* Contribute to an MDT setting.
* Engage in supervision and training.
* Good understanding of medication and other interventions and the evidence that surrounds them.
* Assess and manage risks.
* Skilled in care planning.
