The role
As a Clinical Pharmacist in this practice, you will:
1. Lead medicines optimisation across the patient population
2. Carry out structured medication reviews, particularly for patients with long-term conditions or on complex medications
3. Ensure safe, effective, and cost-efficient prescribing
4. Provide consultations in person, by phone, or video for medicines queries
5. Contribute to QOF, prescribing incentive schemes, and safety audits
6. Work closely with community pharmacies, care homes, and the wider NHS
