Offer description

* To provide support, advice and signposting to the families, friends and carers of people who use the Trust's services.
* To support staff to enable families, friends and carers to be recognised as key partners in care.
* To promote the Triangle of Care (ToC) standards and principles, and other carer specific initiatives to services and carers.


Main duties of the job

* To act as a point of contact for families, friends and carers of those who use our services by providing emotional and practical support (not for the service user).
* To facilitate meaningful carer support groups which provide education and emotional support.
* To undertake carers assessment of need
* To apply for personal budgets for carers based on their needs
* To regularly review carers needs and personal budget
* To promote and advocate carers rights
* To have an increased knowledge of available carer support networks within Stockport and to act as a carer resource for staff and signpost where appropriate
* To collaborate effectively with a range of agencies including PCFT staff, GPs and other health and social care staff
* To attend Triangle of Care Meetings
* To assist and support the ongoing development and implementation of ToC principles and other relevant carer policies, initiatives & strategies.
* To keep and maintain accurate records.
* To assist and support in audits and benchmarking best practice processes
* To share knowledge and expertise with internal and external peers and colleagues
* To organise and actively participate in relevant meetings
* To support the development, delivery and evaluation of training to carers and staff

Person Specification


Education / Qualifications

* Good standard of general education including good numeracy and literacy skills.
* Relevant level 3 Health and Social Care qualification or equivalent experience
* Evidence of recent further development / training equivalent to Level 4
* Commitment to undertake Continuing Personal Development
* Relevant level 4 Health and Social Care qualification


Experience

* Experience of working with carers of individuals with mental health needs
* Experience of working with individuals in distress
* Experience of undertaking carers assessments
* Experience of appling for carers personal budgets


Knowledge

* Knowledge of the Care Act 2014 and other relevant policies
* Knowledge of caring issues and how these impact on individuals
* Awareness of local services.
* An understanding of the mental health system and services


Skills and Abilities

* Good written and verbal communication skills
* Ability to empathise with carers
* Ability to create innovative solutions to help empower carers
* Ability to work unsupervised in a range of settings.
* Ability to acknowledge diversity and promote anti-discriminatory practice/equal opportunities.
* An ability to act calmly in challenging circumstances and to respond in a professional manner.
* Basic IT Skills
* Ability to work flexibly and to prioritise workload
* Ability to work as part of a team


Work Related Circumstances

* Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
* Willing to carry out all duties and responsibilities of the post in accordance with the Trust's Equal Opportunities and Equality and Diversity policies
* Appointments to regulated and controlled activities require an enhanced DBS disclosure.


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
