Overview

As a member of this busy but friendly team you will experience many core aspects of pharmacy practice, including clinical work, commitments to the dispensary and aseptics, and contribute to a 7-day working and on-call service. Pharmacists are part of an engaged and progressive Pharmacy team supporting hospital areas to help staff cope with the population and their complex needs. Working with the multidisciplinary team, key aspects of this clinical post include attending multidisciplinary team meetings, patient education and counselling, clinical audit, medicines safety and medicines information. The department is fully modernised and has ARX robot, EMIS software and has implemented Allscripts Electronic Prescribing in the last 3 years.

We operate with 2 dispensaries on 4 main sites; your rotation will expose you to a full range of clinical activities across Medicine (including medical admissions unit, cardiology and gastroenterology), Surgery (including ICU and Surgical Admissions), Orthopaedics (Wrightington Hospital with experience of anti-TNF therapy and Rheumatology), Oncology and Parenteral Nutrition, Paediatrics, Women\'s Health, Medicines Information, Medicines Safety, and Clinical Trials. Responsibilities

* Provide Specialist Clinical Pharmacy Services to patients as part of the Clinical Pharmacy Team.
* Deliver the highest possible professional standards in dispensaries and oversee dispensary activities in the absence of more senior staff.
* Complete a rotational training plan experiencing major clinical areas within the Trust.
* Ensure compliance with Medicines Law, CQC requirements and internal audit; adhere to medicines management guidance issued by the Trust, GMMMG or national bodies; complete the Clinical Pharmacy Diploma.
* Prioritise patients according to clinical and pharmaceutical need and obtain/validate medication histories.
* Use clinical information to advise clinicians on required changes to medications and suitable alternatives when standard routes are unavailable.
* Analyse medicine charts considering clinical presentation, patient history and clinician plans; endorse in line with Pharmacy standards.
* Ensure VTE compliance and appropriate antimicrobial prescribing per current trust guidance.
* Coordinate with Medicines Management Technicians to ensure patient-specific medicines are ordered; promote reuse of patients\' own medicines where appropriate and manage disposal if unfit for reuse.
* Receive, interpret, clinically check, dispense, perform accuracy checks and hand out prescriptions according to departmental SOPs.
* Communicate with patients and healthcare staff about medicines to improve safety and effectiveness; assist with cost efficiencies via formulary guidance.
* Maintain effective pharmacy stock control; assist in ordering, issuing and receiving medications as needed.
* Provide active risk management to prevent harm to patients and participate in clinical trials with appropriate documentation.
* Complete ward Controlled Drugs audits as scheduled; contribute to developments, projects, audits and surveys as directed by line manager.
* Provide services to Oncology when suitably trained.


Communication and Key Working Relationships

* Assist other health professionals on wards to deliver high standards of care; support advanced Pharmacists in delivering Specialist Clinical Pharmacy Services.
* Coordinate with the pharmacy team to ensure clinically effective services and refer to more senior pharmacists when appropriate.
* Provide ward cover for other clinical teams or for more senior colleagues within the team.


Responsibilities for Finance, Human Resources, Health & Safety and Teaching

* Handle cash, prescription charges and issue relevant receipts; sign orders for pharmaceutical products including controlled drugs in the absence of a more senior pharmacist.
* Organise own work patterns within the framework provided by the line manager; supervise Band 6 pharmacists and pre-registration pharmacists, student pharmacy technicians, Medicines Management Technicians and pharmacy assistants; ensure staff understand and implement procedures; suggest improvements through line management.
* Comply with the Health & Safety at Work Act 1974 by proactively managing risk and reporting incidents, near misses and hazards; ensure patients and staff are advised of medicine risks and use Datix to record and review medicines-related incidents.
* Provide teaching to other healthcare professionals in line with knowledge and experience; teach Band 6 and pre-registration pharmacists in line with GPC and tutors\' expectations.
