Responsibilities:
* Perform daily health assessments on residents and monitor vital signs.
* Administer prescribed medications, ensuring accuracy and timeliness.
* Assist with wound care, including dressing changes and assessment.
* Provide assistance with activities of daily living (ADLs), such as bathing, feeding, and mobility.
* Develop and update individualized care plans for each resident.
* Communicate with residents' families to provide updates and answer any questions.
* Collaborate with physicians, dietitians, social workers, and other healthcare professionals.
* Ensure compliance with infection control protocols and safety standards.
Requirements:
* Education: Registered Nurse (RN) with current licensure in Illinois.
* Experience: At least 1-2 years of experience in a long-term care or nursing home setting preferred.
