About the Role
We are a Specialist Occupational Therapy Team based at Westmorland General Hospital, seeking a Specialist Occupational Therapist to manage a specialist caseload across in‑patient and community settings.
Responsibilities
* Manage a specialist caseload across an in‑patient and community setting, facilitating safe and effective care and hospital discharge.
* Conduct assessment, treatment and goal setting with patients presenting with varied physical conditions.
* Work alongside an integrated community service to deliver patient assessments and collaborative care plans.
* Collaborate closely with colleagues across the MDT and complex discharge team.
* Provide supervision and support to occupational therapy team members and students.
* Engage in service development initiatives and contribute to overall service improvement.
Qualifications
* A Diploma or Degree in Occupational Therapy approved by the Royal College of Occupational Therapists.
* Registration with HCPC as an Occupational Therapist.
* Ability to travel to a variety of venues within the community to deliver patient assessments and attend meetings.
* Knowledge and experience within acute inpatient care and facilitation of safe and effective hospital discharge.
* Previous experience in community‑based rehabilitation services is desirable.
