We are looking for a Band 6 Senior Discharge Planning Nurse to join our developing team working within our Essex community hospitals (Harwich & Clacton), providing a seven day a week service. The role is responsible for supporting ward staff and Case Managers with complex patients and advising on the appropriate discharge pathways. You will be required to have knowledge of the NHS Continuing Healthcare process and completion of appropriate assessment, playing a key role in promoting the discharge to assess pathways. Senior Discharge Planning Nurses contribute to the trust objective of reducing length of stay, reducing readmission rates and the number of delayed discharges.

Responsibilities
* Support ward staff and Case Managers with complex patients and advise on appropriate discharge pathways.
* Promote discharge to assess pathways and ensure knowledge of NHS Continuing Healthcare processes.
* Contribute to reducing length of stay, readmission rates and delayed discharges.
* Organised and experienced in effective multi-agency working.
* Manage a small team and deputise for Discharge leads as required.
Qualifications / Requirements
* Experience as a discharge planning nurse or equivalent in a hospital/community setting.
* Knowledge of NHS Continuing Healthcare processes and discharge planning.
* Ability to work across agencies and manage a small team.
