Salary: £39,027 - £44,463 per annum (pro rata if part-time)

About the Role
As a registered and qualified Social Worker, you will work within a multi‑disciplinary environment, collaborating with social care colleagues and other professionals in the complex field of NHS CHC. You will manage a diverse caseload across all care groups, including older and younger adults with mental health concerns and physical or learning disabilities.
Your responsibilities include undertaking Care Act assessments and reviews for individuals with complex needs who may be eligible for CHC, working closely with health and social work colleagues to determine NHS eligibility, ensuring legal compliance with the Care Act 2014 and the Mental Capacity Act, and promoting independence while supporting service users and families.
What We're Looking For
To be considered, you must have knowledge and working experience of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (July 2022 Revised) and the Care Act 2014.

* Good advocacy skills.
* Sound working knowledge of CHC procedures and processes.
* Ability to apply a strengths‑based approach.
* Understanding of social work issues, legal frameworks, and social services statutory duties.
* Commitment to demonstrate social work values and behaviours.
* Good multi‑disciplinary working skills.
* Excellent communication skills.
* Experience in CHC processes.
* Strong report writing skills.
Essential qualifications: A Social Work qualification and registration with Social Work England.
DBS Check: Enhanced Level with Adults Barred List.
