Overview
The hospital social work team provides support and assistance to adults and their families following admission to hospital. The team works with health professionals to undertake assessments to facilitate smooth, safe and timely transitions from hospital to home or alternative care settings, and supports people in hospital during times of crisis with a person-centred approach within the healthcare system. A Senior Practitioner will assume a leadership role, contributing to the overall effectiveness of the social work team.

Responsibilities
* Provide guidance, support and leadership to the social work team, ensuring effective collaboration and adherence to best practices.
* Supervise Social Workers, providing guidance, support and mentoring to enhance their professional development.
* Hold a small caseload of complex cases which require advanced social work skills.
* Engage in development work and research to contribute to the overall development of the team.
* Lead performance management and quality assurance to ensure best practice, evidence-based decisions, adherence to legislation and BCBC processes.
* Support the team manager in the effective operational functioning of the hospital social work team.
* Collaborate with health professionals and partner agencies.
* Deputise in the absence of the team manager.
Qualifications and requirements
* Demonstrates strong communication skills, enthusiasm, ability to work in a fast-paced environment, proficiency in multidisciplinary working, and the ability to build and sustain relationships with key partners.
* Welsh language ability to greet customers is a requirement for this post.
* Protecting children, young people or adults at risk is a core responsibility of all council employees.
* An Enhanced DBS check with Adults Barred List is a requirement for this post.
* The council’s Hybrid Working Policy applies to this post, providing a framework for working hours between home and the office.
