Summary
Coordinates the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing, and evaluating. Provides leadership for the Healthcare team by working cooperatively with other team members to maintain standards of professional nursing practice in the Home Care setting.
Education and Credentials
* Associates Degree required.
* Graduate of an accredited School of Registered Nursing.
* Current licensure to practice as a Registered Nurse in NYS.
* Valid NYS driver’s license.
Experience
1-5 years of relevant experience, including one year medical/surgical RN experience and one year infusion experience. Verbal and written communication skills necessary for effective patient interaction and reporting. Ability to delegate to LPNs and home health aides. Flexibility, organizational skills, and sound judgment are required.
