Overview
PCN Care Coordinator role at NHS (Bridlington Primary Care Network). The post involves coordinating and navigating care for patients, working with GPs and primary care colleagues to identify and manage a caseload, and delivering personalised care in line with local PCN priorities and health inequalities.
Job responsibilities
* Carry out audits to identify improvements and maximise income in particular IIF, Enhanced Service and claiming procedures.
* Update IT systems to include templates and protocols for changes in service specifications (e.g., IIF changes).
* Work across practices to share good practice, identify information and IT needs, and implement effective systems and training to promote collaboration.
* Establish good working relationships with staff across practices within the PCN to enable effective duties.
* Work with GPs and other primary care professionals within the PCN to identify and manage a caseload of patients and refer people to other health professionals when appropriate.
* Raise awareness within the PCN of shared decision making and decision support tools.
* Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care, using tools such as Ardens and other population health dashboards.
* Design efficient templates, protocols, alerts, and IT tools to support safety, patient care, quality record-keeping, reporting and financial claims.
* Work independently and as part of a team with a proactive approach.
Person Specification
Essential
* Understanding of, and commitment to equality, diversity and inclusion.
* Knowledge of policies and procedures including confidentiality, safeguarding, lone working, information governance and health and safety.
* Experience of working in health, social care or other support roles with direct contact with people, families or carers.
* Experience of working in a multi-disciplinary team environment.
* Confident in using Microsoft applications (Email, Excel, Word, PowerPoint) and sound IT knowledge.
* Ability to organise, plan and prioritise, and work on own initiative; good written and verbal communication skills.
* Willingness to work flexible hours as required; personal accountability and resilience; ability to work collaboratively in a team; ability to communicate effectively with a wide range of people; active listening and empathetic, non-judgemental support.
Desirable
* Knowledge of how the wider NHS works, including PCNs; knowledge of long-term conditions and their complexities (medical, physical, emotional and social).
* Experience of working in primary care; experience as a care coordinator; experience using SystmOne; experience or training in personalised care and support planning; experience with data and reporting systems.
* Higher level qualification such as NVQ in adult care Level 3 or qualifications relevant to health or social care.
Qualifications
* Essential: GCSE or equivalent in English and Maths.
* Desirable: Higher level qualifications relevant to health or social care.
