The salary for this permanent Support Worker job is up to £39,865per annum. Main duties: To support patients/service users in undertaking activities of daily living within care plans. To contribute to the assessment and development of care plans under the direction of a Registered Health or Social Care Professional. To report all accidents and incidents as per policy and demonstrate an awareness of risk management and Quality & Safety processes. To report and escalate any safety, safeguarding or risk concerns to the appropriate member of staff and ensure that action is taken. To build a therapeutic relationship with patients/service users and their support network, communicating with them in a way that respects their views. To use a wide range of communication skills that can be adapted to meet the needs of complex and challenging situations. To demonstrate effective communication skills with all members of the Multi-Disciplinary Team. To provide clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards. To promote high quality care through the role modelling of professional behaviours. To contribute to team success and excellent standards of care by challenging others constructively, upholding HSC's Care Values. To advocate on behalf of patients and service users. To organise own workload and tasks, demonstrating the ability to recognise changing priorities and adjusting plans accordingly. To coordinate and delegate activities to junior colleagues within agreed systems of working. To actively work collaboratively as part of a Multi-Disciplinary Team with a range of professional from other service areas who may be participating in the care of a patient/service users. To demonstrate the ability to develop new knowledge and skills, applying this to practice and sharing with colleagues as appropriate. To actively contribute to an effective learning environment and support others in their learning, participating in formal supervisor roles as appropriate.
