Overview

Trainee Advanced Clinical Practitioner (Critical Care Outreach) – Hampshire Hospitals NHS Foundation Trust. Closing date: 22 October 2025.


Main duties of the job

1. Undertakes comprehensive clinical assessment of a patient's condition, requests and performs tests/investigations as appropriate, utilising specialist knowledge to initiate and manage effective clinical treatment plans.
2. Critically analyses, evaluates, and synthesises different sources of information to assess and manage patient care, applying the principles of diagnosis and clinical reasoning that underpin clinical judgment and decision making.
3. Interprets results of diagnostic tests/investigations and makes recommendations for treatment or adjusts the patient’s clinical treatment plan as appropriate and within the scope of own role.
4. Recognises own limitations and effectively seeks appropriate help/assistance when working within the service’s scope of practice.
5. Performs specialist clinical skills and/or invasive interventions within scope of practice and under appropriate supervision until deemed competent to perform independently, such as venesection, cannulation, urinary catheterisation, administration of IV fluids/medications and/or injection therapy (this list is not exhaustive and is profession specific; refer to your service specific scope).

Job responsibilities

An exciting opportunity has arisen to join our 24/7 Critical Care Outreach Team at Hampshire Hospitals. Candidates will be able to work cross-site in our two acute Hospitals in Basingstoke and Winchester. The successful candidate will have recent experience in Critical Care Outreach and be enthusiastic about extending their skill and knowledge in all four pillars of advanced practice. The team attends any patient in ward areas who is deteriorating and uses advanced skills to work with ward medical and nursing teams, providing early intervention to prevent organ failure. Work closely with Critical Care Consultants and senior nurses to support early discharge of patients from Critical Care units, and aid transfer of patients from ward and ED areas to Critical Care in a safe and timely manner. Work across the trust to recognise early deterioration by also responding to calls in the Martha’s Rule system from ward staff, the public, or patients. We are involved in informal and formal training provision, from bedside teaching of clinical skills to classroom teaching of Trust RRAPID deterioration course, and in conjunction with HSS University a Level 7 deteriorating adult module. Analysis and QI projects on data collected from our patient group to constantly improve care. Candidates would be expected to line manage and develop junior members of the Outreach team, improving standards and developing our future ACP workforce. Person Specification


Training and Qualifications

* Completion of Healthcare Degree (min 120 credits at Level 6 or evidence of Masters study)
* Completion of Recognised Critical Care Course
* ALS Provider
* Tracheostomy training and completed competencies
* Critical Care Transfer Training
* Maths and English Level 2 or recognised equivalence from NARIC process
* Completed NORF Enhanced level Competencies
* Completed HTPA course
* Independent Non-medical prescribing qualification
* Completion of CCOT foundation programme
* Completion of Step 1-2 Critical Care Competencies


Experience and Knowledge

* Significant experience within Critical Care outreach
* Evidence of supervisory responsibilities
* Evidence of working independently, prioritising and using own initiative safely
* Evidence of participation in audit and research and development
* Evidence of previous extended practice


Skills and Ability

* Knowledge of CCOT/Critical Care at BSc level or equivalent
* Understanding of quality assurance within the field of Critical Care
* Ability to learn and assimilate information quickly
* Evidence of student supervision and mentorship


Other specific requirements

* Evidence of effective clinical leadership and communication skills
* Ability to establish and evaluate new processes
* Ability to work and communicate within the multi-professional team
* Demonstrates good diplomatic and negotiating skills
* Awareness of wider Trust/national initiatives and pressures
