Overview

The Frailty & Outreach Service is a proactive, community-based team within the PCN, working to improve health outcomes for adults living with frailty or long-term conditions. We bring together nursing staff, allied health professionals, and support staff to provide holistic, joined-up care.

* Frailty & Outreach: visiting adults at home who may be at risk of worsening health outcomes due to frailty or complex needs. The team takes a proactive approach to assessment and care planning, helping people stay well, independent, and avoid unnecessary hospital admissions.
* Long-Term Conditions (LTC): delivering annual reviews for housebound patients on behalf of GP surgeries, ensuring people with conditions such as diabetes, COPD, and cardiovascular disease continue to receive safe, effective, and person-centred care.


Main duties

The post holder will be an integral member of the Frailty & Outreach Service, combining administrative expertise with direct clinical support. A key part of the role is contributing to the delivery of LTC provision for housebound patients through accurate management of patient spreadsheets, timely reporting of review outcomes, and proactive liaison with GP surgeries to support smooth service delivery.

About Us

East & Central Brighton Primary Care Network (PCN) is a collaboration of nine GP practices: Arch Healthcare, Ardingly Court Surgery, Broadway Surgery, Park Crescent Health Centre, Pavilion Surgery, Regency Surgery, St Peters Health Centre, Wellsbourne Healthcare CIC, and Woodingdean Medical Centre. Together, these practices serve a diverse population across Brighton, working collectively to strengthen local healthcare provision and improve patient outcomes. The PCN delivers extended services beyond core GP provision, including proactive frailty and long-term condition management, health outreach, and additional patient-facing initiatives designed to meet the needs of the community. Collaboration within the PCN enables teams to share knowledge, streamline pathways, and provide integrated care for patients with complex needs. Job Description


Job responsibilities

Administrative Support

* Receive and process referrals for frailty and LTC services, ensuring timely allocation and follow-up.
* Maintain accurate and confidential patient records on EMIS/SystmOne.
* Prepare documents, correspondence, reports, agendas, and minutes as required.
* Order and monitor office/clinical supplies using agreed procurement processes.

Service & Team Support

* Contribute actively to the life of the Frailty & Outreach team, supporting both administrative and clinical work.
* Help develop and improve reviewing systems and organisation within the service.
* Support audit, data collection, and produce reports (including Excel-based reporting).
* Take accurate minutes at meetings and follow up on actions.
* Provide cross-cover for other administrative staff as required.

LTC Support

* Contribute to the delivery of LTC provision for housebound patients by accurately managing patient spreadsheets.
* Ensure reviews are appropriately documented and outcomes are reported in a timely manner.
* Liaise proactively with GP surgeries to support smooth service delivery and maintain strong working relationships.
* Provide administrative support to clinicians delivering LTC reviews, including preparing information, recording data, and following up on referrals.

Clinical / Patient-Facing Support (with training and supervision)

* Assist with collecting and processing patient samples.
* Undertake simple clinical tasks such as blood pressures.
* Chaperone clinicians during home visits and clinics.
* Provide reassurance, information, and signposting to patients and carers.
* Engage confidently with people from a wide range of backgrounds, including those experiencing disadvantage or complex social needs.


Person Specification

Qualifications

Essential

* Good standard of literacy and numeracy (e.g. GCSE English and Maths or equivalent).

Desirable

* NVQ Level 2/3 in Health & Social Care, Business Administration, or equivalent.

Other Requirements

* Flexible approach to working across PCN teams as required.
* Willingness to travel between sites and support home visits as needed.
* Car and driving license essential.

Experience

Essential

* Previous experience in an administrative role (healthcare or equivalent).

Desirable

* Experience working in a healthcare environment, especially community or primary care.
