The Opportunity

ECC Adult Social Care is transforming from a focus on long-term care support to a model with four key principles: prevention, early intervention, enablement, and safeguarding. The role enables people to access information and tools to live independently and respond to growing service demand within ECC’s financial constraints.

Working with Social Workers and Occupational Therapists, you will deliver bespoke intervention work in accordance with care plans, and may manage a caseload, including risk assessment through direct work within a quality assurance process. A focus of the role is safeguarding vulnerable people in line with ECC’s statutory obligations.

Experience with vulnerable adults or an education to Level 3 (RQF) is desirable. You should be passionate about engaging with and providing community support, and able to work with partners as required.

Accountabilities

* Assess social care needs and provide support to vulnerable adults to enable healthy, independent living; maintain high professional standards contributing to continuous improvement and the four key principles.
* Contribute to the development of care and support plans that secure solutions meeting individual needs.
* Work collaboratively with partners, including the placement team, to support individuals to commission care and support from appropriate providers; develop Direct Payments as required.
* Work within statutory legal frameworks, policy and guidance and determine when thresholds for statutory intervention are reached, in line with the Care Act 2015 (e.g., MCA/DoLs/CHC).
* Highlight safeguarding issues and implement plans to safeguard a vulnerable adult.
* Build strong relationships with clients, families and carers, provide practical advice and support, and advocate where necessary to enable independent living.
* Collaborate across the service and with internal and external partner agencies to deliver joint objectives and improve outcomes for vulnerable adults.
* Provide information and advice on involvement/access to third-party agencies relevant to client needs (e.g., benefits, immigration, housing).
* Attend meetings, reviews, case conferences and court hearings to provide information, insight and evidence, and provide reports where appropriate.
* Individual and shared targets and objectives are defined annually within the performance management framework.

The Experience You Will Bring

* Experience within a social care setting; an NVQ/BTEC Level 3 or equivalent is desirable.
* Evidence of continuing professional development and knowledge in a relevant professional area.
* Experience building strong relationships with adults, their families and carers in times of challenge and crisis.
* Experience undertaking direct, activity-based work with adults.
* Knowledge of legislative and policy guidance applicable within Social Care settings.
* Proven track record of building working relationships with partners and external agencies.
* A certificate in First Aid and/or knowledge of Sign Language is desirable.
