Overview
Senior OT / Pathway determiner - Community Therapy Services role in Rushcliffe, working in the community as an Occupational Therapist. This opportunity supports a forward thinking and innovative service within Rushcliffe Community Therapy Service.
Responsibilities
* Work closely with Physiotherapists, Assistant practitioners, support staff, district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.
* Contribute to an integrated community service offering clinical triage, rehabilitation, falls management, palliative care support and support timely discharges from acute Trust partners.
* Work autonomously as well as part of the multi-disciplinary team, liaising with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care.
* Visit patients within their own homes using a goal-orientated focus to maximise independence, safety and quality of life.
* Cross-site working and community visits as required.
Requirements / Qualifications
* Full UK driving licence and vehicle for business use (reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010).
