With an increasing complexity of patients in our community, our team provides seamless holistic nursing care to ensure patients receive the right care. We are looking for individuals who can be part of our fast-moving, innovative and ambitious teams, dedicated to supporting our communities to manage their own health both at home and in our dedicated clinics.
You will be responsible for providing skilled nursing care to patients across the locality and work flexibly with multidisciplinary teams across our service to support our patients' journey.
Job responsibilities
* Full-time post to work within the West Bridgford Community Nursing team.
* Provide community nursing care following a robust holistic assessment to enable patients to be cared for in their own homes and clinic settings.
* Facilitate prevention of unnecessary hospital admission/re-admission by reducing the effects of ill health and disability and maximizing individuals' potential for independence.
* Work flexibly in the community to support evolving and developing services, supporting the team to deliver high-quality integrated care.
* Work independently, but also be a team player.
* Participate in a robust induction and competency-based development package; newly qualified staff will be offered a preceptorship programme.
* Community experience is preferred but those with appropriate transferable skills will be considered.

Person Specification
Qualification
* Professional Qualification (General) to degree level or equivalent.
* Maintains current registration with the NMC.
* Demonstrates continued professional development to maintain professional registration.
Experience
* Demonstrates satisfactory clinical expertise for the banding.
* Specialist and area specific experience.
Skills
* Ability to work as part of a team.
* Ability to be flexible and handle conflict issues.
* Good skills in assessment intervention and evaluation of patients nursing care.
Knowledge
* Knowledge and understanding of current legislation and understanding of clinical risk assessments.
* Knowledge of the nursing 6 c's.
* Awareness of national and local issues impacting nursing care.
Training
* Willingness to undertake new clinical practices, education and training that will enhance service delivery.

Disclosure and Barring Service Check
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Applicants will be required to pay for their DBS check. The cost will be deducted from salary over the first two months of employment. Enrol for DBS Update Service at £16 per year.
