An opportunity has arisen for a driven and innovative Assessment and Review Worker to join People’s Services within Contact Cares. This large, busy, multi-disciplinary team serves as the single point of access for health and social care referrals. The team includes both qualified and unqualified Social Care, Nursing, and Therapy staff. We offer planned and urgent assessments, including a 2-hour response under Urgent Community Response Criteria.
The Contact Cares team also provides initial contact, care planning, onward referrals, and signposting services for a wide range of adult clients across health and social care. Our work focuses on prevention, early intervention, and reducing hospital admissions and readmissions. The work is varied and engaging.
The team operates from 8:00 am to 10:00 pm, 7 days a week, including Bank Holidays. This position involves shift work on a rota basis.
