Community navigator- specialist role with older adults

Overview

As the Community Navigator, your role will aim to address the rising needs of older adults in East and South Cambridgeshire by connecting them with local services, community groups, and emotional support to reduce isolation, enhance mental health, and support independent living.


Responsibilities

* Provide person-centred support to clients to help them live well and achieve their individual outcomes.
* Link people to local services and community groups.
* Provide advice, information, guidance and signposting to navigate available support.


Qualifications

* NVQ level 2 (or equivalent) in Health and Social Care or willingness to work towards this.
* Experience of working with individuals to assess their needs and produce plans to meet outcomes.
* Values alignment with Making Space and Disability Confident.

Salary and Hours

Salary: £14.35 per hour
