Job responsibilities Attend team meetings and care planning meetings as required, ensuring follow- up action is taken. Participate in the development of staff within the team. Communicate effectively with all staff, clients/patients and carers in accordance with Trust / Directorate standards to promote high quality of care. Ensure client / patient confidentiality and understand the need to share information as needed. Take a keen interest in own personal professional development. Attending adequate training to ensure issues relating to service users and their families are assessed and addressed in relation to the policy, safeguarding Children. To act as Shift Coordinator within the Home Treatment Team To provide a range of interventions for service users to effectively manage any identified risks, to support them with their acute mental health needs and to support them to resolve crisis situations To develop a thorough understanding of deliberate self-harm, suicidality and management strategies using evidence based practice and NICE guidelines To observe and practice within the legal requirements of the Mental Health Act 1983. To contribute to the establishment and revision of policies, procedures and practice guidelines relating to the work of the Home Treatment Team and the interface between other Mental Health Services.
