About us

Role Purpose

This is a frailty‑led paramedic role within an innovative single practice Primary Care Network, focused on delivering proactive, preventative and patient‑centred care to frail and vulnerable patients, particularly those in care homes or at risk of deterioration.

The postholder will play a central role in reducing avoidable hospital admissions, supporting patients to remain in their usual place of residence and delivering anticipatory and coordinated care across the system.

Alongside this, the role will contribute to acute and same‑day demand, providing responsive clinical assessment and triage for the wider practice population.

The role requires a clinician who is comfortable working autonomously, while also contributing meaningfully within a multidisciplinary team environment.

Foundry is made up of 4 sites and has over 29,000 patients across our practices. We have many teams that help provide the most effective treatment to our patients and can support our GPs.
